In consideration of our involvement and participation in the “DCL-2023” above said club sport activity, we hereby voluntarily assume all risks of personal injury or personal property damage which might be associated with participation and involvement in following sport club activity. I further voluntarily remise, release and forever discharge the DCL-2023/WSS members, its successors, assigns, officers from all injuries, losses or damages of any kind whatsoever suffered by me because of my voluntary use and participation in this sport. I agree to follow all instructions, rules and regulations of DCL-2023 while engaged in sport club activities.
I further hereby represent that I do not have any medical impairment, disease, physical liability or injury which would prevent our participation in the said program; and that we have medical insurance that covers our participation.
Further, in consideration of our participation in the activity, I agree on behalf of myself and my heirs and anyone else acting on my behalf to release, indemnify, and hold harmless, and covenant not to sue the DCL-2023/WSS, and all DCL-2023/WSS officers and personnel paid or volunteer associated or connected with the said program, DCL-2023/WSS  and the  League Tournament Organizing Committee for every claim, liability or damage of any kind caused by the negligence of the State of Kentucky, The DCL-2023/WSS personnel involved or otherwise which may result from our participation in the said program.
I HAVE READ THIS WAIVER AND ASSUMPTION OF RISK AGREEMENT, I FULLY UNDERSTAND ITS TERMS, I UNDERSTAND THAT I HAVE GIVEN UP LEGAL RIGHTS BY SIGNING IT, AND I SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
