I, the below signed, am aware that playing or practicing any athletic activity can be a dangerous activity involving many RISKS OF INJURY. I understand that the dangers and risks of playing or practicing in this sport include but are not limited to serious injury or impairment to organs or aspects of the body and general health and well-being of my ward.
 
To the best of my knowledge, “Select Player” is in good health and suffers no disability or condition which renders his/her participation in the sport or other athletic activity medically inadvisable, or otherwise limits his/her ability to participate in such sport or athletic activity without restriction.
 
I hereby authorize the coach (or other Woodley Youth Cricket Academy personnel) to obtain on my behalf, first aid, emergency medical care, or if necessary, admission to an accredited hospital, when such care is necessary for the treatment of any injuries that my son/daughter may sustain while participating in any activity associated with Woodley Youth Cricket Academy, including practices, games and travel. I agree to be responsible for all medical charges incurred by any hospitalization or treatment rendered pursuant to this authorization.
 
In consideration of accepting this registration and to the extent permitted by law, I, for myself or as Parent or Guardian, hereby voluntarily assume all the risks and hazards incidental to participation in the activities and transportation to and from the activities. I release absolve and indemnify Woodley Youth Cricket Academy, and parent volunteers, SCCA and all concerned authorities, volunteers, school district & organizations and sponsors from all risks and hazards associated with the activities.
 
I give permission to the Woodley Youth Cricket Academy, SCCA and all authorities, organizations to take photographs of me or my children while participating in this activity for use in future publicity and understand that I will not receive any compensation for such use.
 
Woodley Youth Cricket Academy is not a registered club and it is organized by volunteers, parents and Coach. Coach reserves the right to decline registration to anyone and can stop participation in the program for any reason.
 
I certify that I have read and understand this Waiver and Release as it applies to me and to any minors for whom I am signing.
