In consideration of my participation in activities conducted and/or sponsored by CLNJ, I hereby:
- Acknowledge that such activities are potentially hazardous and pose risk of injuries that may be significant and that I assume such risks.
- Agree to release, hold harmless and not to sue CLNJ, its officials, Board of Directors, representatives, and agents on account of any injury and claims of injury to person while participating in or traveling to and from CLNJ activities. I also agree that signing this form shall also bind my spouse and relatives, to release, hold harmless and not to sue CLNJ, its officials, Board of Directors, representatives, or agents.
- Permit CLNJ to use, for any lawful purpose, photographs, motion pictures, recordings, or any other media of CLNJ activities in which I appear.
- Acknowledge and accept that automobiles used in conjunction with CLNJ activities shall not be covered under the property damage liability insurance provided by CLNJ. I understand that I am required to carry auto liability insurance as required by the state where the automobile is registered.
- Attest that I am in good physical health and have no existing physical disability, illness or condition of any type that may be aggravated by the physical requirements of participating in CLNJ activities.
- Acknowledge that I have read and fully understand CLNJs by-laws and code of conduct documents. I agree to abide by the laws, rules and regulations as set forth by CLNJ and accept any disciplinary action that may be handed down by CLNJ officials pursuant to any behavior on my part that may, at CLNJ’s sole discretion, warrant such an action.
- Acknowledge and accept that submission of this form is an integral requirement and precondition of participation in any CLNJ activity.
I acknowledge that I am over the age of 18 years, have read this agreement and fully understand its terms; and I understand that I will be given up substantial rights by signing it. I have signed it freely and without any inducement or assurance of any nature, and I intend it to be a complete and unconditional release of liability to the maximum extent permitted by law and agree that if any portion of this agreement is held to be invalid, the balance, notwithstanding, shall continue in full force and effect.
I hereby release, discharge, covenant not to sue, and agree to indemnify, save, and hold harmless the Cricket League of New Jersey, its officials, Board of Directors, representatives, and agents (“releasee”), from all liability, claims, demands, losses, or damages on the player’s account caused, or alleged to be caused, in whole or in part by the negligence of the releasee or otherwise, including negligent rescue operations.
I further agree that if, despite this release, I, or anyone else on my behalf makes a claim against the releasee, I will indemnify, save, and hold harmless each of the releasees from any litigation expenses, attorney fees, loss liability, damage, or any cost that may occur because of such a claim.
