AMATEUR ATHLETIC WAIVER AND RELEASE OF LIABILITY ***(READ BEFORE SIGNING)***

In consideration of being allowed to participate in any way in ADULT CRICKET LEAGUE with TCL and related events and activities, the undersigned acknowledges, appreciates, and agrees that:

• The risk of injury from the activities involved in this program is significant, including the potential for permanent paralysis and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist; and,
• I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and,
• I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately; and,
• I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS TCL their officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law.
• I willingly agree that to the stated terms that any media (photos or videos) captured during TCL events/games/programs/practices can be used for promotional purposes on Website/Social Media/Print.
• EXPRESS ASSUMPTION OF RISK: UNDERSIGNED hereby acknowledges and understands that the World Health Organization has declared COVID-19 a worldwide pandemic. COVID-19 is extremely contagious and spreads mainly from person-to-person contact. Based on currently available information and clinical expertise, older adults and people of any age who have serious underlying medical conditions may have a higher risk for severe illness from COVID-19. There have been recommended guidelines and preventative measures put in place to reduce the spread of COVID-19 in society; however, TCL CANNOT GUARANTEE that the participant will not become exposed to or infected with COVID-19.  Furthermore, participation by participant in the Events could increase participant’s risk of contracting COVID-19. By signing this Agreement, UNDERSIGNED acknowledges the extremely contagious nature of COVID-19 and voluntarily assumes the risk that the participant may be exposed to or infected with COVID-19 from participation in the Events, and that such exposure or infection may involve the RISK OF SERIOUS INJURY, ILLNESS, PERMANENT DISABILITY AND/OR DEATH. UNDERSIGNED understands that the risk of becoming exposed to or infected with COVID-19 by UNDERSIGNED’s participation in the Events may result from the actions, omissions, or negligence of others and/or UNDERSIGNED, including, but not limited to, the RELEASEES (as defined below). UNDERSIGNED hereby expressly assumes all such risks and dangers whether presently known or unknown;and,
• I willingly agree to comply with the guidelines percribed by the league relative to the COVID.

I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.

 

Play your part and observe TCL’s Respect Code of Conduct for players at all times.

On and off the field, I will:

• Adhere to the Laws of The Game
• Display and promote high standards of behavior
• Promote Fair Play
• Always respect the match officials’ decisions
• Never engage in public criticism of the match officials onfield
• Never engage in offensive, insulting or abusive language or behavior
• Never engage in bullying, intimidation or harassment• Speak to my team-mates, the opposition, and my coach/manager with respect
• Never Smoke or Consume Alcohol in and around the playing field.
• Remember we all make mistakes.
• Win or lose with dignity. Shake hands with the opposing team and the referee at the end of every game.

I understand that if I do not follow the Code, any/all of the following actions may be taken by my club, league or organization: I may:

• Be required to apologize to team-mates, other teams, referees or coach.
• Receive a warning from the coach
• Receive a written warning from the TCL Enforcement committee 
• Be required to attend a meeting with the Enforcement Committee
• Be dropped or substituted
• Be suspended from training
• Be required to serve a suspension
• Be fined
• Be required to leave the club

Also:

• The league could impose a fine or other sanction against my team for my violation of any of the above.
• The undersigned player agrees to abide by the TCL Code of Conduct & Rules document in www.trianglecricketleague.org
